Individual Support Plans
Where members or volunteers have a known allergy, medical condition or any other support needs, you should have a conversation about how you might and will support them when attending any of your sessions.
The Individual Support Plans provide information that is used to support members and volunteers during YAC sessions, help plan for activities to avoid allergens or to provide additional support, and to help them in an emergency.
Individual support plans may be temporary (e.g. a broken leg / pregnancy) or longer term (e.g. allergies, asthma or a specific access need).
Creating an Individual Support Plan
Individual Support Plans can be used for members and volunteers alike.
The completed Individual Support Plan is treated as private and confidential and will only be used as described above.
Please be aware that YAC Leaders may be required to share aspects of this information with other volunteers and professionals (e.g. medical practitioners) only as it relates to the health and wellbeing of the individual.
We have created template letters for the parents/carers of YAC members to explain what the Individual Support Plan is and how it is used to support their child:
We have two versions of the support plan for members and volunteers:
Medication
If a member or volunteer carries medication that requires additional training and/or instruction, e.g. an EpiPen, the Leaders at your branch will ask you to provide this information and will take instruction from the emergency services in an emergency. If additional, formal training is required, please email yac@yac-uk.org with this request.
If a member or volunteer provides you with medication for storage, you should identify a safe, secure and, in an emergency, accessible storage solution and complete a Medication Log:
Please Note: this is different to the provision of sunscreen or the provision of First Aid – please read the guidance on the Incident and Emergency and Procedures page.
