YAC's standard membership form. This form has been updated to meet with the requirements of the new General Data Protection Regulations that come into force on 18th May 2018. Please ensure that you update all of your members' records onto this new form before May 2018, and securely destroy all previous information that you hold for your members.
The new form includes emergency contact details for parents/carers, and space for details of any medical conditions and additional needs. The form also includes a section to provide photographic permission. Importantly, it now requests parents'/carers' consent to store and use personal data.
Please note: this form should be checked with parents/carers annually, and updated where necessary.
GDPR consent slip
Use this addendum slip to ensure that your members' forms are GDPR compliant. It asks for specific consent to store and use your members' personal information, and should be attached to their membership form.
Only use this slip if you have recently updated your membership forms with your members (i.e. between January and April 2018) and don't want to ask your members' parents/carers to complete a whole new form.
Our new Membership form (PDF version) is GDPR compliant, so when updating forms in the future, please use this version.
This form should be used to record details of any members or leaders in your YAC with specific medical needs, for example allergies that need the use of an epi-pen. As with membership forms, healthcare plans should be checked and updated annually (where appropriate)
Third-party permission form for images/film
This form can be used to collect permission to take photographs and record film of your members during projects that you are undertaking with other organisations or partners.
For example, you can use this form if your YAC is taking part in a project to create video diaries of an excavation, where the film will be used both on your own website (or the main YAC website) and the website of the group organising the excavation.
