Branch Safety Forms

Here are all the forms you need for the day to day running of your Branch meetings.

Risk Assessment Form

In order for you to be covered by YAC Insurance you must complete a risk assessment for each activity you plan to do, as well as for the venue you are using. Risk assessments should be kept for ten years. See your YAC Leaders’ Handbook (Part Four) for further information on Risk Assessments.

Accident Form

Use this form to keep a record of any accident, no matter how small, that occurs during a Branch meeting. Once completed these forms should be kept in a secure place for three years. For further information on accident procedures, see your YAC Leaders’ Handbook (Part Four).

In the case of major accidents this form should be sent to YAC HQ in York and contact should be made with the Head of YAC or CBA Director. See contact us page for details.

Incident Form

This form should be used to record any situations where bad behaviour has occurred. It should report the events accurately but not include personal opinions. Please note medical incidents should be recorded on the Accident form above. More information about managing behaviour can be found in your YAC Leaders’ Handbook (Part Two).

Referral Form

Use this form to record any situations where: you are concerned a child may be at risk from abuse; a child has disclosed abuse to you; you are concerned about the behaviour of a Leader; an allegation of abuse has been made against a Leader. Remember you must refer, you must not investigate. See your YAC Leaders’ Handbook (Part Four) for further information on Child Protection.
