These are the forms that you will need to give to Parents and Guardians.

YAC Letter to Parents/Guardians

An introductory letter to parents and guardians about what YAC is.

YAC Parent/Guardian Permission Form

Use this form for any trips and outdoor visits your Branch goes on. You must have a signed parent/guardian permission slip for each YAC member on the trip.

Medical and Emergency Contacts Form

You should hold copies of a completed Medical and Emergency form for each member of your Branch. These should be updated annually, and old forms destroyed. Please note the completed forms should be kept securely and only approved Branch Leaders or Assistants can have access to them.

Photographic Permission Form

All young people attending your meetings should have a completed photographic permission form, even if there has been no permission given (ie all the answers are no!) These should be updated annually and the old ones destroyed. Please note the completed forms should be kept securely and only approved Branch Leaders or Assistants can have access to them.

Parent/Guardian Guidelines

You should have copies of these guidelines to hand at every Branch meeting. Any parents/guardians or other non-YAC registered volunteers who are present at a session should be given a copy of these guidelines to keep. The guidelines outline the types of task that non-YAC registered adults may undertake and give a brief outline of YAC’s child protection policy.

Please note that any adult attending Branch meetings in order to assist with the running of the event (and intending to supervise children who are not their own) should be signed up as a registered YAC volunteer.
