YAC's standard membership form. It includes emergency contact details for parents/carers, details of any medical conditions and additional needs. The form also includes a section to provide photographic permission.

This form should be used to record details of any members or leaders in your YAC with specific medical needs, for example allergies that need the use of an epi-pen. As with membership forms, healthcare plans should be checked and updated annually (where appropriate)
Third-party permission form for images/film
This form can be used to collect permission to take photographs and record film of your members during projects that you are undertaking with other organisations or partners.
For example, you can use this form if your YAC is taking part in a project to create video diaries of an excavation, where the film will be used both on your own website (or the main YAC website) and the website of the group organising the excavation.
