Incident reporting form
Use this form to record any concerns that you have about a child or young person under the age of 18; whether they are your own concerns, a report from someone else, or a disclosure of abuse from the child themselves.
Please email the completed form to the Council for British Archaeology's Designated Safeguarding Officer, Mike Heyworth at firstname.lastname@example.org with the subject line: "INCIDENT FORM – RESTRICTED".
This form is for recording the details of any accidents that happen at your YAC sessions where First Aid is given.
Session planning template
This template will help you to plan your YAC sessions, and may be particularly useful if you're new to YAC. If you use this, you do not need to do a separate risk assessment. There are some completed session plans below for guidance.

Risk assessment template
A form for assessing risk to make sure your YAC sessions are run safely. If you have planned your session using our session planning template you do not need to complete a separate risk assessment.
Behavioural incident form
This form is for keeping a record of any incidents of difficult behaviour at your YAC sessions.
This form will guide you through the process for closing your YAC club, if you need to.
YAC Sponsorship form
You can use this form to collect monies when undertaking a sponsored event in aid of YAC. If you need any help or ideas for fundraising events, do get in touch with us at YAC HQ. Thank you for helping to raise money for the club!
Financial record template
You can use this simple template to help you to manage your club's finances. You might also like to set up a spreadsheet. If you need any advice about recording and managing your club's finances, drop us an email to email@example.com
Remember that we need to receive information about the finances of all traditional YAC clubs as part of our annual report. We must report to the Charities Commission about the monies that you receive and pay out from your club's bank account over the course of the financial year.
